See how technology supports the Alameda County Social Health Information Exchange

Alameda County, California launched a social health information exchange to integrate and aggregate information across sectors including data from jail systems, HMIS systems, managed care organizations, and community-based organizations. The data integration has enabled the creation of a community health record that provides a 306-degree longitudinal view of clinical and non-clinical utilization to enable providers to better coordinate care for high-risk consumers facing medical and social challenges.
